We generally collect the following information:

  • Information we receive when you use our Services. We collect Web-Behavior and Mobile Device Information via cookies and other tracking technologies when you use and access our website, mobile apps, and other services.
  • Information you share directly with us. We collect and process your information when you use the D2M Application to request a visit, create an account, login to the account, complete surveys, comment on articles, use messaging features, potentially with other third-parties and Customer Support.

How We Use Information

We generally process Personal Information, for the following reasons:

  • To provide our Services. We process Personal Information in order to provide D2M’s Services, including account creation, login authentication, communication, facilitating visits, providing users with access to health information, processing payments, and insurance support.
  • To analyze and improve our Services. We work to improve and provide new tools and Services, fix bugs, analyze usage, and assess marketing campaigns.

Sharing Your Information

Privacy Practices

THIS NOTICE DESCRIBES HOW YOUR MEDICAL INFORMATION MAY BE USED AND DISCLOSED AS WELL AS HOW YOU CAN ACCESS YOUR INFORMATION.

WHO FOLLOWS THIS NOTICE

This notice describes the privacy practices of the independent professional health care entities (“Practices”) contracting with DOCTOR2ME, Inc. The Practices operate as affiliated covered entities. DOCTOR2ME, Inc. does not provide medical services, nor does it refer or recommend any physician or provider.

YOUR RIGHTS

  • Electronic or paper copy of medical record: You may request an electronic or paper copy of your medical record. We will provide it within required time frames.
  • Correction: You may request corrections to your health information. We will review and respond within 60 days.
  • Confidential communications: You may request alternative communication methods. We will comply with reasonable requests.
  • Limit use/sharing: You may request limits, though it may affect your care or conflict with laws.
  • Accounting of disclosures: You may request a list of disclosures within the last six years.
  • Copy of this notice: You may request a paper copy at any time.
  • Choosing a representative: A guardian or power of attorney may exercise your rights.
  • File a complaint: Contact us or file with HHS OCR (www.hhs.gov/ocr/privacy/hipaa/complaints/).

YOUR CHOICES

For certain health information, you may tell us your choices:

  • Sharing information with family, friends, or others involved in your care
  • Sharing for marketing purposes
  • Fundraising efforts by Doctor2Me
  • Apple healthcare API (Doctor2Me does not connect to Apple APIs)

Exceptions:

  • Disaster relief situations
  • Emergency treatment if unconscious or at serious risk

OUR USES AND DISCLOSURES

We typically use or share your health information in these ways:

  • Other health professionals: To coordinate treatment.
  • Internal improvement: To improve services (with deidentified data).
  • Billing: Though services are self-pay, information may be provided to health plans.

SPECIAL CATEGORIES OF INFORMATION

We restrict sharing of psychotherapy notes, genetic testing, developmental disabilities, HIV/AIDS, and substance abuse treatment information per applicable laws.

You acknowledge receipt of this notice and consent to electronic delivery.

CONSENT TO EMAIL AND ELECTRONIC COMMUNICATIONS

You consent to email, SMS, or electronic messages for treatment-related purposes, understanding these are not fully secure. Do not use for emergencies.

CONSENT TO USE OF TELEHEALTH

  1. During telehealth, provider and patient are in different locations. Benefits and risks exist.
  2. Provider information is available in the D2M Application. Contact in emergencies: 866-362-1499, intake@doctor2me.com, or doctor2me.com.
  3. Telehealth may be used for exams, diagnosis, treatment, prescriptions, and communication.
  4. You may withdraw consent at any time.
  5. Information may be integrated into your record and secured, but risks remain.
  6. No third-party sharing without consent, except as required by law.
  7. The Practice is not responsible for technical failures.
  8. In-person evaluation is required for certain prescriptions.
  9. In a medical emergency, call 911.
  10. All questions must be answered to your satisfaction.

CONSENT TO HEALTHCARE MARKETING COMMUNICATIONS

CONSENT TO MARKETING COMMUNICATIONS: You consent to receive healthcare-related communications by electronic, SMS, or mail.

CONSENT TO DISCLOSURE OF INFORMATION FOR MARKETING: D2M and affiliates may share your information with partners for healthcare or related services. Consent may be revoked at any time.

PATIENT ACKNOWLEDGMENT

You acknowledge that you:

  • Have read and accepted the Practice’s Patient Agreement
  • Received and reviewed the Notice of Privacy Practices
  • Consent to E-messages
  • Read and understood the Consent to Use of Telehealth

If a patient is unable to consent, you agree as a legally authorized representative.
